Retrograde Urethrogram (RGU)
Definition
Retrograde urethrogram is a diagnostic procedure performed most commonly in male patient to diagnose urethral pathology.
Indications
Trauma to urethra
Urethral stricture
Suspected urethral fistula
Difficulty in urine passes
Risk Factors/Complications
Urinary tract infection
Uretheral trauma
Intravasation of contrast(Due to use of excessive pressure)
Urethral bleeding
Contrast
The water soluble contrast urographin 60% is used which is diluted with normal saline in 1:3 ratio. (Ionic contrast – diatrizoate)
Equipments
X ray unit
Fluoroscopy with spot film device
Urographin
Foley’s catheter
Syringe
Surgical Gloves
Anaesthetic jelly and Betaden.
Preparation
The procedure should be explained to patient before undergoing to procedure.
All metallic ornaments should be removed specially in the pelvis region.
The patient is asked to micturate prior to procedure
Procedure
The patient lies supine on the table and the plain kub film is taken.
Then the patient is slightly tilted legs position.
Using sterile technique first of all clean the tip of the penis with betadine.
Then the catheter is inserted into the tip of the penis after applying an anaesthetic jelly and slightly inflate the balloon with sterile water to fix the catheter.
Then the small amount of contrast(10-15ml) is injected into the urethra and the spot images are taken
The patient should be oblique to visualize full length of urethra.
Exposure should be made when the contrast is injected.
Filming : -
Plain KUB film (Including kidneys ureters bladder and urethral areas.)
After Contrast : -
Pelvis AP Oblique
The images are taken in both oblique projection when the contrast is injected into the urethra.
